HOW TO REGISTER:
To register at the practice just pop in to see us, ideally with two forms of ID.
You will be asked to complete a registration form and you will be given a basic health questionnaire to complete. Once you have registered with the practice you may be invited to attend for an initial health check and to discuss your medications. If you are on repeat prescriptions, please ensure you have 3-4 weeks of medication from your previous GP to see you over the registration period.
Alternatively you can download a GPR registration form and email it to us at fife.f21558newburgh@nhs.scot Please note we will respond by email attaching our medical questionnaire to capture information about your repeat medications and pre-existing medical conditions.
CHANGE OF PERSONAL DETAILS
If you change your name, address or telephone number, please notify us immediately so that we can amend our records. If you move out with the practice area, we will have to ask you to register with another practice nearer to your new home. You can view our practice boundaries here
